What Is A 64-8 Form?
The 64-8 Form is the most important piece of paperwork we need to claim your tax rebate for you. This is the document that lets us act as your official “agent” with HMRC.
Why do you need it?
Without form 64-8, the taxman wonโt even talk to us about you. Data protection and GDPR laws mean that HMRC canโt discuss your affairs with someone else without your permission.
How do I complete a 64-8?
A 64-8 is easy to complete online or by downloading it and sending it to HMRC by post to:

HM Revenue and Customs
BX9 1AN
Your clients can useย form 64-8ย to authorise you to act on their behalf for:
- individual, partnership, and trust tax affairs
- requests toย HMRCย for information on the High Income Child Benefit Charge to help calculate the charge
- tax credits
- Corporation Tax
- PAYE for employers
- Construction Industry Scheme (CIS) for contractors
- VAT
It is important to only tick the relevant areas of taxย you would like your agent to help you with.
How do I complete it?
We need you to do the following:
- If we have sent you the form online, print the form off or let us know if you need us to send a hard copy for you (in the post).
- Fill in your name
- Fill in your address
- Fill in your National Insurance number
- Sign it
- Post or drop it back to us (we cannot accept scanned or pdf forms as we always need to send the hard copy by post back to HMRC).
